I was in remission when I came out, and, for various reasons had never before considered myself disabled, and, did not/was not at the time. So, luckily didn't have that to deal with as well.
From what I've heard (yeh, it's only heard, tho it was from a conference full of LGBT disabled people), a lot of the disabled movement/scene is pretty homophobic.
And, in my experience, the LGBT scene is not too inclusive/acessible/open-minded. So, due to the above mentioned factors,(and quite a few others-*) I think it is significantly more difficult to come out if you are also disabled.
*What about the fact that a number of disabled folk live in homes that offer them little, or no privacy, and/or support to engage in sexual relationships/activity. (coz disabled people aren't supposed to have sex, are we? Not even straight sex, let alone Gay sex, huh!).
*Also, getting out and about is generally much more difficult if you're disabled. And/or acessing the media (e.g t.v/mags for ppl who r blind), esp.alternative media (e.g LGBT) that does not always have the funds to make them accessible to all.
This fact, I think, leads to quite a few of us missing out on parts of our culture. And, if you're not exposed to ideas/culture, then it's impossible to take them on, i,e the idea of coming out, being out+ proud etc.
But anyway, all of this is just theory to me, coz like I said I was not disabled at the time I came out. Better to hear it from the horses mouth, eh? (is that the right phrase? I'm not sure?).

I think every thing you said there is very valid .. there just isnt the funds about to change things though .. and there isnt the education for people in minority groups as there is for people in the majority. Its very hard .. but thats why GLAD started .. its (hopfully) an assessable site thats openminded!!!
